Consent and release terms

Authorized organizations. Souls of The Arts, United Souls Gospel Choir, Uniquely Gifted Liturgical Dance Ministry, Global Media Ministries, Yesha Ministries Worship Center, and their authorized event, production, broadcast, and ministry partners may make and use the media covered by my selected option.

Use and distribution. Authorized media may be edited, combined with other content, reproduced, displayed, streamed, published, and distributed in any lawful format or channel for event documentation, ministry, education, publicity, fundraising, and promotion.

No compensation or approval. Neither my child nor I will receive payment, royalties, or other compensation, and I will not have a right to inspect or approve finished content.

Public live-recording setting. Organizers will make reasonable efforts to honor limited or no-consent selections, but incidental background appearance in crowd or wide-angle footage may be difficult to prevent.

Duration and withdrawal. Permission remains effective unless withdrawn in writing. Withdrawal applies only to future use and cannot require removal of media already published, distributed, broadcast, archived, or included in completed materials.

Authority and release. I certify that I am the child's parent or legal guardian and have authority to grant this permission. To the extent allowed by law, I release the authorized organizations from claims arising from uses permitted by this form, except claims based on their negligence or willful misconduct.

Child assent (recommended for ages 13–17)

I understand that photos, video, audio, or livestream content may include me, and I agree to participate under the permission selected by my parent or guardian.
